To get started, you’ll need:
  • A copy of the organization’s IRS determination letter as a non-profit.
  • A list of the Board of Directors/Trustees and other members of the organization’s governing body.
  • Audited financial statements or a copy of the last tax filing with the IRS.
  • The organization’s current operating budget and project budget if applicable.
  • Board of Directors/Trustee resolution of Authority for this application.
  • A completed application form.
